Customer Deposit Form

    Applying online for a MyLife MyFinance savings or investment account is simple.

    To complete this application you'll need some form of identification, such as:

    • Your passport details
    • Your Australian driver's licence details
    • Your Medicare details
    • A utility bill showing your name and current address
    • Your CSF statement

    You may also be verified using online sources such as the Australian Electoral Roll or the Sensis White Pages Database.

    Evidence of the above may be required prior to final approval.

    If you are aged 16 or 17 and are unable to complete the online identification, please complete one of the following options:

    OPTION 1.

    Scanned copies of one of the following documents to info@mylifemyfinance.com.au

    • Birth Certificate

    • Proof of Age Card (both sides)

    • Existing bank account statement with name and address shown and a copy of your Medicare Card

    OPTION 2.

    We will forward the Statement by Referee Form to your nominated email address which can be completed and the original returned to us at:

    Saving your application

    If you don't have all the information to complete your application, you can save your application at any time and pick up where you left off later. To save your application, simply click SAVE at the bottom of the page, enter your email address and we'll send you a unique passcode to access your application in the future.

    Submitting your application

    Once you have completed your online application, your details will be sent securely to our team. We'll open your account and send you the details within 1 business day.

    Like a copy for your records?

    You can keep a copy of your application simply by clicking the 'print' button on the last page of the application.
